ECMOand Hyperbaric Oxygen Therapy Used Simultaneously in U.S. First

Virginia Mason Franciscan Health (VMFH) has reported a pioneering U.S. medical case in which Extracorporeal Membrane Oxygenation (ECMO) and Hyperbaric Oxygen Therapy (HBOT) were delivered simultaneously to a critically ill patient.

According to VMFH, the 27-year-old patient suffered from severe carbon monoxide poisoning accompanied by critical cardiac dysfunction. Because conventional hyperbaric oxygen treatment would have presented significant risks in such a condition, the medical team used ECMO to provide circulatory and respiratory support while administering HBOT.

Combining Advanced Critical Care with Hyperbaric Medicine

ECMO provides temporary heart and lung support by oxygenating and circulating a patient’s blood when the cardiovascular or respiratory system is severely compromised.

HBOT, meanwhile, delivers 100% oxygen under increased atmospheric pressure, allowing oxygen to reach tissues more effectively. It is used clinically for a range of conditions, including severe carbon monoxide poisoning, decompression sickness, and selected difficult-to-heal wounds.

In this case, the two treatment modalities were used simultaneously to address both the patient’s critical cardiac condition and carbon monoxide poisoning.

VMFH reported that the patient subsequently showed significant recovery and is continuing to recover.

Preparation Behind the Breakthrough

The treatment was supported by months of preparation, including:

  • Testing ECMO and other critical-care equipment for use inside a hyperbaric chamber
  • Conducting simulations and safety evaluations
  • Cross-training physicians, nurses, respiratory therapists and ECMO specialists
  • Developing protocols for complex critical-care procedures in a pressurized environment

The case demonstrates the importance of equipment compatibility, chamber safety, clinical protocols and multidisciplinary collaboration when advanced medical technologies are combined.
